ABSTRACT

Objective:To evaluate the autogenous arteriovenous fistula(AVF) insufficiency by ultrasound monitoring of brachial artery resistance index(RI).Methods:The brachial artery RI and blood flow volume(FV) data of 828 patients who underwent color-Doppler ultrasound detection after AVF in the General Hospital of Western Theater Command from January 2019 to June 2021 were retrospectively analyzed. The patients were grouped according to the adequacy of clinical dialysis, including 668 patients in the group with normal AVF function and 160 patients in the group with insufficient AVF function. The general information and ultrasonic measurement parameters were compared between the two groups. The correlation between brachial artery FV and RI was analyzed. The evaluation of brachial artery flow RI for AVF insufficiency was analyzed by ROC curve.Results:There were statistically significant differences between the two groups in brachial artery RI and FV(both P<0.001). The results of Pearson correlation analysis showed that brachial artery FV was negatively correlated to RI ( r=-0.657, P<0.001). The area under ROC curve for assessing AVF function by brachial artery RI was 0.970, with 95% CI was 0.955-0.986, the optimal cut-off value was 0.665, and the sensitivity and specificity were 0.888 and 0.955, respectively. Conclusions:Brachial artery RI in patients with AVF insufficiency is significantly higher than that in patients with normal AVF function. The optimal cutoff value of brachial artery RI can be used as an evaluation parameter for rapid screening of AVF function.

ABSTRACT

ObjectiveOn the basis of sensory evaluation, the changes of volatile components in gecko before and after processing were compared, and the odor correction effect of different processing methods of gecko was discussed. MethodRaw products, fried yellow products, vinegar processed products, wine processed products, talcum powder scalding products and white wine sprayed products after scalding talcum powder of gecko were prepared, and 10 odor assessors were invited to evaluate the 6 samples in turn by sensory evaluation. Headspace solid-phase microextraction-gas chromatography-mass spectrometry (HS-SPME-GC-MS) and relative odor activity value (ROAV) were used to analyze the key odor components, and multivariate statistical methods were used to analyze the difference of volatile components between raw and processed products of gecko. Taking water-soluble extract and protein contents as internal indicators, sensory evaluation score and content ranking of differential components as external indicators, and assigning a weight of 0.25 to them respectively, the comprehensive scores of raw products and processed products of gecko were calculated to evaluate the odor correction effect of each processing method. ResultThe average sensory evaluation scores of the raw products, fried yellow products, vinegar processed products, wine processed products, talcum powder scalding products and white wine sprayed products after scalding talcum powder of gecko were 1.6, 5.2, 6.2, 6.1, 7.2 and 8.0, respectively. ROAV results showed that key components affecting odor of gecko were 2-ethyl-3,5-dimethylpyrazine, isovaleraldehyde, trimethylamine, 1-octen-3-ol, n-octanal, nonanal, 2-methylnaphthalene, γ-octanolide, 2-heptanone and phenol. Principal component analysis (PCA) significantly distinguished raw products from processed products. Orthogonal partial least squares-discriminant analysis (OPLS-DA) results showed that there were 16, 13, 16, 16, 16 differential components between raw products, fried yellow products, vinegar processed products, wine processed products, talcum powder scalding products and white wine sprayed products after scalding talcum powder of gecko. Among these differential components, there were 4 common components, namely, the contents of different odor components (2-methylnaphthalene and 2-ethyl-p-xylene) decreased, while the contents of different flavor components (2-decanone and 2,3,5-trimethylpyrazine) increased. The comprehensive scoring results showed that the odor correction effect of each processed products was in the order of talcum powder scalding products>wine processed products>vinegar processed products>fried yellow products>white wine sprayed products after scalding talcum powder. ConclusionTalcum powder scalding is a better method to improve the odor of gecko, and it can provide an experimental basis for the processing of gecko to correct the odor.

ABSTRACT

Objective@#To investigate the influencing factors of nosocomial infections among patients undergoing radiotherapy, so as to provide insights into nosocomial infection control among radiotherapy patients. @*Methods@#The malignant tumor patients receiving radiotherapy in Yiwu Central Hospital from 2019 to 2020 were selected. Their demographic features, radiotherapy and nosocomial infections were collected through the eletronic medical records. The factors affecting nosocomial infections were identified among radiotherapy patients using a multivariable logistic regression model.@*Results@#Totally 553 malignant tumor patients receiving radiotherapy were enrolled, including 268 men ( 48.46% ) and with a mean age of ( 60.31±13.66 ) years. The mean duration of hospital stay was ( 28.67±14.32 ) days, and the mean radiotherapy cycle was ( 3.63±1.89 ) weeks. A total of 46 patients were detected with nosocomial infections, with prevalence of 8.32%. The top three sites with the highest prevalence of nosocomial infections included the lower respiratory tract ( 14 cases, 30.43% ), oral cavity ( 10 cases, 21.74 ) and urinary tract ( 8 cases, 17.39% ), and 42 bacterial strains were isolated, with Gram-negative bacteria as the predominant strains ( 25 strains, 59.52% ). Multivariable logistic regression analysis showed that radiotherapy cycle of 4 weeks and longer ( OR=3.857, 95%CI: 1.896-7.846 ), radiotherapy exposure ( OR=3.254, 95%CI: 1.561-6.783 ), arteriovenous catheterization ( OR=2.165, 95%CI: 1.144-4.098 ), urinary tract intubation ( OR=6.028, 95%CI: 1.841-19.734 ) and poor nutritional status ( OR=3.933, 95%CI: 1.493-10.360 ) were associated with nosocomial infections among radiotherapy patients.@*Conclusion@# Radiotherapy cycle, exposure to radiotherapy, arteriovenous catheterization, urinary tract intubation and poor nutritional status are factors affecting nosocomial infections among radiotherapy patients.

ABSTRACT

Objective:To explore the clinical efficacy of double opposing rhomboid flap in repairing facial skin defects.Methods:From January 2020 to December 2020, 30 cases of facial skin lesions were removed in the Department of Plastic and Aesthetic Surgery, the Affiliated Hospital of Qingdao University, including 12 males and 18 females, aged 14-65 years, with an average age of 34.2 years. The diameter of the facial skin defect wound was 0.5-2.0 cm. The patients with facial skin defect were repaired with double opposing rhomboid flap. All patients were followed up for 3-12 months.Results:The incisions of 30 patients were healed in one stage, and double opposing rhomboid flaps survived. Following-up for 3 to 12 months showed that the operation area was flat, the incision scar was not obvious, the texture and color of the operation area and the surrounding skin matched well, the surrounding organs were not deformed, the lesions were not recurrent, and the cosmetic effect was satisfactory.Conclusions:The double opposing rhomboid flap is an effective method to repair quasi-circular facial skin defects, which is worthy of clinical application.

ABSTRACT

Objective:To examine the relationship between conflict management model, perceived social support and psychological distress in psychiatric nurses, and to analyze the mediating effect of conflict management model in the relationship between perceived social support and psychological distress in psychiatric nurses.Methods:In November 2019, a total of 208 nurses in Shandong Mental Health Center were investigated with General Demography Questionnaire, Nurses' Interpersonal Conflict Handling Form Scale, Kessler Psychological Distress Scale and Perceived Social Support Scale.Results:The total score of psychological distress of psychiatric nurses was 22.74±7.71, and the level of psychological distress was negatively correlated with the integrated conflict management model and perceived social support ( r values were -0.329--0.266, P<0.001). The most commonly used conflict management model of psychiatric nurses was "integration". The integrated conflict management model had a partial mediating effect in perceived social support and psychological distress, and the mediating effect accounted for 20.2% of the total effect. Conclusions:Psychological distress of psychiatric nurses is at a moderate to severe level, and integrated conflict management model and perceived social support play an important role in reducing psychological distress of psychiatric nurses. Nursing administrators should carry out relevant intervention studies to improve nurse-patient conflict and guide nurses to adopt integrated conflict management model to reduce psychological distress and improve mental health level.

ABSTRACT

Objective:To understand the relationship between psychiatric nurses' communication ability, personality characteristics and conflict management patterns, and to provide a basis for training psychiatric nurses' conflict management ability courses.Methods:A convenient sampling method was used to investigate and investigate 208 nurses in the first-line clinical hospitals of a third-class psychiatric hospital, used the nurses′ clinical communication ability scale, the nurse′s interpersonal conflict management morphology scale, and the simplified version of the Chinese Big Five Personality Questionnaire.Results:Integration dimension (18.67±2.58) points, concession dimension (12.93±2.43) points, escape dimension (14.05±2.02) points, dominance dimension (14.49±3.06) points, and integration dimension and gender ( t value was -2.57, P=0.01)), education ( F value was 3.76, P=0.02), technical title ( F value was 3.03, P=0.02), position ( t value was 2.08, P=0.03), whether to participate in nurse-patient communication training ( t value was 2.54, P=0.02). Nurses with pleasant personality could better deal with interpersonal conflict ( r=0.361, P<0.01), and the nurse′s personality had a direct effect on the form of interpersonal conflict ( P=0.036, 95% CI 0.107-0.422), the nurse′s communication ability plays a part of indirect effect ( P=0.031, 95% CI 0.001-0.063). Conclusion:The form of interpersonal conflict management of psychiatric nurses was closely related to communication ability and personality characteristics. Nursing managers could carry out targeted training to improve the conflict management ability of nursing staff.

ABSTRACT

OBJECTIVE:To compare the difference in the antioxid ant effect of fresh Rehmannia glutinosa ,dried R. glutinosa , R. glutinosa preparata during ancient characteristic processing and its polysaccharides before and after processing on aging model rats,and to provide reference for the processing of R. glutinosa . METHODS :The sample of R. glutinosa preparata was prepared according to ancient characteristic method. During the processing ,the fresh and dried R. glutinosa samples were retained. Then crude polysaccharide were extracted from fresh R. glutinosa and Rehmanniae radix preparata by water extraction and alcohol precipitation. Totally 96 rats were divided into blank group (water),model group (water),positive control group [vitamine C ,100 mg/(kg·d)],fresh R. glutinosa group [ 700 mg/(kg·d)],dried R. glutinosa group [ 135 mg/(kg·d)] ,Rehmanniae radix preparata group [ 135 mg/(kg·d)],fresh R. glutinosa polysaccharide group [ 1 400 mg/(kg·d),by the weight of fresh R. glutinosa ] and Rehmanniae radix preparata polysaccharide group mg/(kg·d),by the weight of Rehmanniae radix preparata] , with 12 rats in each group. Except for blank group ,other 制。E-mail:zhouyan1221@163.com groups were given D-galactose [ 125 mg/(kg·d)] on neck and back to induce sub-acute aging model. At the same time ,they were given relevant medicine in tragastrically,once a day ,for consecutive 56 days. After last admin istration,the liver ,brain,kidney,spleen,heart and thymus indexes were determined. The total antioxidant capacity (T-AOC),superoxide dismutase (SOD)activity,catalase(CAT)activity and MDA content in serum , liver,brain and kidney were determined. RESULTS :Compared with blank group ,organ indexes of rats in the model group were decreased significantly (P<0.05 or P<0.01);T-AOC,SOD activity and CAT activity in serum ,brain,liver and kidney tissue were decreased significantly (P<0.01),while MDA content increased significantly (P<0.01). Compared with model group ,the organ indexes of brain ,liver and kidney ,SOD activity in serum and kidney of fresh R. glutinosa group were not significantly increased (P>0.05);kidney index ,T-AOC in serum and brain ,SOD activity in serum ,liver and kidney tissue were not significantly increased in the dried R. glutinosa group(P>0.05);kidney index ,T-AOC in serum and cerebral tissue ,SOD activity in serum were not significantly increased in fresh R. glutinosa group(P>0.05);other organ indexes ,T-AOC,SOD activity and CAT activity in serum and tissues were increased significantly in other groups (P<0.05 or P<0.01),while MDA content in serum and tissues were decreased significantly in all administration groups (P<0.05 or P<0.01). Compared with fresh R. glutinosa group,T-AOC in serum was decreased significantly in dried R. glutinosa group(P<0.01),and there was no significant difference in other indexes (P>0.05);kidney and spleen indexes of rats in Rehmanniae radix preparata group were increased significantly (P<0.05),T-AOC in renal tissue ,SOD activity in serum ,cerebral tissue and renal tissue ,CAT activity in cerebral and liver tissue were increased significantly (P<0.05 or P<0.01),while MDA in cerebral and liver tissue were significantly decreased (P< 0.01). CAT in cerebral tissue and liver tissue of rats in Rehmanniae radix preparata group were significantly higher than those in positive control group (P<0.01). Compared with fresh R. glutinosa polysaccharide group ,spleen and renal indexes of rats in Rehmanniae radix preparata group were increased significantly (P<0.05 or P<0.01),T-AOC,SOD activity and CAT activity in serum and cerebral ,liver,renal tissues were increased significantly (P<0.05 or P<0.01). T-AOC and CAT activity of cerebral , liver and renal tissues in Rehmanniae radix preparata group were all significantly higher than those in positive control group (P< 0.05 or P<0.01). Objective@#To investigate the relationship between psychiatric nurses' self-efficacy, self-acceptance and coping style, and to explore the intermediary role of psychiatric nurses′ self-efficacy in self-acceptance and positive coping style.@*Methods@#Totally 813 clinical nurses in 6 prefectural and municipal psychiatric hospitals in Shandong Province were investigated with general information questionnaire, simple coping style scale and self-acceptance questionnaire, and the results were analyzed.@*Results@#The scores of self-efficacy and positive coping styles of psychiatric nurses were (2.40 ±0.54) and (2.09 ±0.47) points.The difference was statistically significant (t=-15.072, 12.76, P < 0.01), the total score of self-acceptance was (42.28 ±4.91) points, the score of self-acceptance factor was (21.76 ±3.60) points, the score of self-evaluation factor was (20.53 ±3.36) points. There was no significant difference compared with the domestic norm (P>0.05). Positive coping style was positively correlated with self-efficacy, self-acceptance and self-evaluation (P < 0.01), negative coping style was negatively correlated with self-acceptance and self-evaluation (P<0.01). Self-acceptance and self-efficacy were important predictors of coping style, Self-efficacy plays an intermediary role in self-acceptance and coping style, and the mediating effect was 0.155, 95%CI (0.117-0.202).@*Conclusion@#Improving self-efficacy of psychiatric nurses was one of the effective ways to improve coping style of nurses.

Objective@#To investigate the status quo and influencing factors of work burnout of psychiatric nurses, and to provide theoretical basis for formulating intervention strategies for work burnout of psychiatric nurses.@*Methods@#A questionnaire was conducted on 812 nurses selected from 6 tertiary A psychiatric hospitals in Shandong province as the research object by stratified random sampling, and the questionnaire was conducted by using the Maslach Burnout Inventory (MBI), the Kessler Psychological Distress Scale (K10) and Emotion regulation questionnaire (ERQ).@*Results@#There were significant differences in job burnout among psychiatric nurses with different educational background, age, nurse-patient conflict and income level. There were significant differences in the dimensions of job burnout among psychiatric nurses with different titles (P< 0.05), There were significant differences in emotional exhaustion (F=8.417, P=0.000) and depersonalization level (F=7.918, P=0.000) between psychiatric nurses with different frequency of nurse-patient conflict.There were significant differences in affective exhaustion (F=6.426, P=0.002) and personal accomplishment (F=3.444, P=0.032) between psychiatric nurses with different monthly income levels. Psychiatric nurses' age, working years, psychological distress, cognitive reappraisal, inhibition of expression were associated with job burnout (r=0.075~0.570) . Regression analysis revealed that the factors impacting emotion exhaustion (R2=0.374, F=81.627, P=0.000) and depersonalization (R2=0.240, F=65.014, P=0.000) were psychological distress, inhibition of expression, cognitive reappraisal, conflict between nurses and patients. The factors impacting diminished personal accomplishment (R2=0.199, F=68.332, P=0.000) were psychological distress, cognitive reappraisal and age.@*Conclusions@#The work fatigue of psychiatric nurses was at the moderate level. Emotional regulation and psychological distress were the influencing factors of job burnout.Nursing administrators in psychiatric department should pay attention to the improvement of nurses' sense of burnout, improve the ability of nurses' emotional management, cultivate positive mood and behavior pattern, reduce anxiety and depression and so on, so as to effectively improve the mental health level of nurses in psychiatric department.

ABSTRACT

Objective To analyze the patients′ psychological resilience level of postpartum pelvic floor dysfunction and to explore the influencing factors, in order to provide the scientific basis for patients′psychological intervention. Methods A cross-sectional survey was conducted on 208 patients of postpartum pelvic floor dysfunction by using the Connor-Davidson Resilience scale(CD-RISC), Positive and Negative Affective scales(PANAS) and Medical Coping Modes questionnaire(MCMQ, which includes "confrontation","acceptance-resignation"and"avoidance"). Results The score of psychological resilience of 208 patients was (53.64 ± 12.29)points, differences in age, family monthly income, living locations, delivery methods and delivery times reached a statistically significant level(F=15.287,5.731,t=-2.571,-15.911,-2.922,all P<0.05). Patients′psychological resilience level had significant positive correlation with positive emotions(P<0.01)and"confrontation",and it had negative correlation with"acceptance-resignation"(P<0.01).Age,living locations, delivery methods, positive emotions and "acceptance- resignation" are the forecast factors of psychological resilience, which can explain 69.6% of total variation. Conclusions Patients of postpartum pelvic floor dysfunction have low levels of psychological resilience, which associated with positive emotions and medical coping modes. Therefore, nursing staffs should pay attentions to the patients′ psychological status, develop individualized psychological guidelines,implement psychological intervention and health education,in order to improve the level of patients′resilience and improve their living quality.

ABSTRACT

Objective To investigate the present status and influencing factors of psychological distress among psychiatric nurses, and provide theoretical basis for intervention measures. Methods Totally, 812 psychiatric nurses in six psychiatric hospitals of Shandong province were selected to complete the Kessler Psychological Distress Scale (K10), Perceived Organizational Support(POS), Simplified Coping Style Questionnaire(SCSQ) and Emotion regulation questionnaire(ERQ). Results The mean scores of psychological distress among psychiatric nurse was (21.29±8.11) points,70.3%of psychiatric nurses had psychological distress. There were significant differences in psychological distress among psychiatric nurses with different age (F=6.886,P=0.001), working years (F=4.403,P=0.004), professional title (F=6.597,P=0.000) and experience of nurse-patient conflict (F=7.228,P=0.000).Psychological distress was negatively related to cognitive reappraisal(r =- 0.093), positive coping style (r =- 0.163) and perceived organizational support(r =- 0.379), while psychological distress was positively related to expressive suppression(r=0.160) and negative coping style(r=0.258)(all P<0.01). Regression analysis revealed that the factors impacting psychological distress were positive coping style, perceived organizational support, expressive suppression, negative coping style and nurse-patient conflict (R2=0.224,F=46.611,P=0.000). Conclusions Psychiatric nursing managers should pay attention to the cultivation of emotional management ability of nurses and the ability to deal with conflict,encouraging them apply positive thinking cognition and positive coping style to deal with negative events in the work,creating a supportive work environment in order to improve the level of mental health.

Methods Totally, 812 psychiatric nurses in six psychiatric hospitals of Shandong province were selected to complete the Kessler Psychological Distress Scale (K10), Perceived Organizational Support(POS), Simplified Coping Style Questionnaire(SCSQ) and Emotion regulation questionnaire(ERQ). Results The mean scores of psychological distress among psychiatric nurse was (21.29±8.11) points,70.3%of psychiatric nurses had psychological distress. There were significant differences in psychological distress among psychiatric nurses with different age (F=6.886,P=0.001), working years (F=4.403,P=0.004), professional title (F=6.597,P=0.000) and experience of nurse-patient conflict (F=7.228,P=0.000).Psychological distress was negatively related to cognitive reappraisal(r =- 0.093), positive coping style (r =- 0.163) and perceived organizational support(r =- 0.379), while psychological distress was positively related to expressive suppression(r=0.160) and negative coping style(r=0.258)(all P<0.01). Regression analysis revealed that the factors impacting psychological distress were positive coping style, perceived organizational support, expressive suppression, negative coping style and nurse-patient conflict (R2=0.224,F=46.611,P=0.000). Conclusions Psychiatric nursing managers should pay attention to the cultivation of emotional management ability of nurses and the ability to deal with conflict,encouraging them apply positive thinking cognition and positive coping style to deal with negative events in the work,creating a supportive work environment in order to improve the level of mental health.

ABSTRACT

Objective To investigate the levels of cancer related fatigue (CRF) and the correlations between CRF and serum inflammatory factors and hypothalamus-pituitary-adrenal (HPA) axis in patients with gastrointestinal cancer.Methods The CRF level was assessed by brief fatigue inventory (BFI).The level of C-reactive protein (CRP) was measured by immunoturbidimetry, and the level of cortisol was measured by electrochemiluminesence.The levels of interleukin (IL)-6, IL-1β, tumor necrosis factor-α (TNF-α), adrenocorticotropic hormone (ACTH) and norepinephrine (NE) were measured by enzyme-linked immunosorbent assay (ELISA).Results The average total score of CRF was 3.15±1.93, and the degree was mild to moderate, which was positively correlated with the CRP (r=0.321, P=0.000), TNF-α (r=0.265, P=0.000), NE (r=0.174, P=0.015) and ACTH (r=0.257, P=0.000), but was not correlated with the cortisol (r=0.033, P=0.652).Eastern Cooperative Oncology Group (ECOG) score (t=8.081, P=0.000), education (t=-4.244, P=0.000), treatment (t=4.563, P=0.000), time from diagnosis to sampling (t=3.453, P=0.001) and CRP (t=2.837, P=0.006) were important factors of CRF.Conclusion The CRF status is common in gastrointestinal cancer patients.The CRF is correlated with the NE and ACTH of HPA axis.Medical staff should pay attention to the inflammatory factors and hormone levels to improve the fatigue status and the quality of patients.

ABSTRACT

Objective@#To investigate the impact of intensified maintenance therapy on the prognosis of children and adolescents with advanced lymphoblastic lymphoma (LBL) .@*Methods@#Retrospective analysis on the treatment results of children and adolescents with stage Ⅲ and stage Ⅳ LBL who underwent BFM-NHL-90/-95 regimen without prophylactic radiotherapy. The intensified therapy group included the patients admitted from 1998 to 2005, while others were classified as the non-intensified therapy group. Patients in the intensified therapy group were intravenously treated with "etoposide phosphate plus cytrarabine" and high-dose methotrexate alternately per 2.5-3 months in addition to the oral chemotherapy with 6-mercaptopurine and methotrexate during the maintenance phase.@*Results@#A total of 187 LBL patients were enrolled. The rates of 5-year event free survival were (76.9 ± 5.8) % and (77.9 ± 4.3) % (χ2=0.249, P=0.617) respectively, in the intensified therapy (n=52) and the non-intensified therapy groups (n=135) , while the rates of 5-year overall survival of them were (78.8 ± 5.7) % and (79.8±4.1) % (χ2=0.353, P=0.552) , respectively. Stratified by stage, immunological type as well as risk stratification, the rates of long-term survival were similar between the two groups. During the maintenance phase, the rates of grade Ⅲ and Ⅳ myelosuppression in the intensified therapy and the non-intensified maintenance groups were 55.8% and 18.5%, respectively (χ2=25.363, P<0.05) .@*Conclusion@#Intensified maintenance therapy failed to improve the prognosis of patients with advanced LBL.

ABSTRACT

Objective To explore the relationship among psychological resilience, self-efficacy and core competence in psychiatric nurses. Methods Totally 237 psychiatric nurses in a psychiatric hospital in Shandong province were recruited and assessed with general information questionnaire, Connor-Davidson Resilience Scale, General Self-Efficacy Scale and Competency Inventory for Registered Nurse. Results The total core competence of psychiatric nurses was (2.59 ± 0.66) points, at a moderate level. Psychological resilience were positively corrected with self- efficacy (r=0.454,P=0.000) and core competence (r=0.647, P=0.000), self-efficacy were positively corrected with core competence (r=0.487, P=0.000). Psychological resilience partially mediate the relationship between self- efficacy and core competence, It′s mediating effect was 44.8%. Conclusions Psychological resilience plays an important role to core competence in psychiatric nurses.Nursing managers should carry out related training, improve the psychological resilience and self-efficacy of psychiatric nurses, and comprehensively improve the level of core competencies.

Fatigue, which is one of the most commonly reported symptoms in cancer, can negatively impact the functional status and the health-related quality of life of individuals. Although awareness and study of CRF have grown in recent years,the biological mechanisms and risk factors that induce CRF remain unclear.This paper systematically reviews the available evidence on the biological mechanisms and the risk factors to guide the development of targeted, individualized interventions for cancer-related fatigue.

ABSTRACT

Objective: To investigate the effect of gross total resection on the local control and survival of patients with stage IV neuroblastoma (NB) and analyze the extent of surgical resection of primary tumors that affects patient survival. Methods: A total of 96 patients with stage Ⅳ NB who were admitted to the Sun Yat-Sen University Cancer Center between January 2000 and December 2011 were analyzed. The patients were treated with combined-modality therapy, including chemotherapy, surgery, and/or radiotherapy. The patients were divided according to the extent of surgical resection of primary tumor into the following groups: group A, biopsy or tumor removal of less than 50% of the primary lesion; group B, incomplete resection of more than 50% but less than 90% of the lesion; group C, removal of more than 90% of the lesion; and group D, complete resection with or without macroscopic residual tumors. The survival rates of each group were analyzed. Results: The median age of the 96 patients was 4.4 years, ranging from 1.2-18.8 years. The overall 3-year progression-free survival (PFS) and overall survival (OS) of the total patients were 32.8% and 36.7%, respectively. A total of 24 cases were assigned in group A, 10 in group B, 23 in group C, and 39 in group D. Subgroup analysis revealed that the 3-year PFS rate was 17.5% for group A, 20.0% for group B, 45.1% for group C, and 40.5% for group D. The PFS rates were not statistically significant-ly different between groups A and B (P=0.352) and between groups C and D (P = 0.792). However, the OS was higher in groups C and D than that in groups A and B. The 3-year PFS rates were 42.2% and 17.8% for groups C and D (P<0.001), respectively. Conclu-sion: Resection extension of more than 90% of the primary tumor combined with chemotherapy and (or) radiation therapy can improve the survival of patients with stage Ⅳ NB. However, this treatment modality does not affect the treatment outcomes for minimal gross tu-mor residuals.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the long-term survival of children and adolescents with lymphoblastic lymphoma (LBL) treated by a modified NHL-BFM-90 protocol.</p><p><b>METHODS</b>From March 1998 to November 2010, 107 untreated patients with LBL (age <18 years) were enrolled and stratified into three groups (R1, R2 and R3), according to the stage of disease and response to induction chemotherapy. All patients received different intensive chemotherapy regimens based on a modified NHL-BFM-90 protocol. Total treatment duration was 2 years.</p><p><b>RESULTS</b>Of the 107 patients, 79 were boys and 28 were girls, with a median age of 10 years (range 2.5-18 years). Six patients (5.6%) were stage I/II, 101 (94.4%) stage III/IV. The R1, R2 and R3 groups accounted for 5.6%, 71.0% and 23.4%, respectively. 75.7% of the patients had T-LBL, and 24.3% was B-LBL. At a median follow-up duration of 60 months (range 1-186 months), 24 patients died. The 5-year event-free survival (EFS) and overall survival (OS) were 75.5% and 77.8 % for all patients, 100.0% and 100.0% for group R1, 84.5% and 87.5 % for R2, 44.0% and 44.0% for R3, 72% and 73.5% for T-LBL, 86.4% and 88.5% for B-LBL, respectively. Myleosuppression was the major toxicity and need aggressive management.</p><p><b>CONCLUSION</b>The modified NHL-BFM-90 protocol is an effective therapy for children and adolescents with LBL in low and intermediate risk. T-LBL had the similar outcomes as B-LBL did. The patients in high-risk group had a poor survival and new protocols are needed.</p>
